ADVERTISEMENT FOR BIDS

Section A

City of St. Joseph 2018 Repair and Resurfacing Program

The City of St. Joseph invites sealed bids from qualified companies interested in
providing paving/resurfacing services in support of the City’s street maintenance
efforts. The contractor selected will be responsible for preparing, grading,
leveling, cleaning, tacking and placing a bituminous chip seal surface overlay on
streets in the City of St. Joseph. This bid request is structured as:

(1) Scope of Work with approximate square yardage of pavement (chip seal) area of
12,330, and:
For scope of work, the bid should include traffic control, associated vegetation matter
removal, clearing, cleaning, grading, and any necessary saw cutting, subgrade
preparation, base installation, etc.

Bidders shall place their bid in a sealed envelope along with a bid bond and proofs
of insurance. The outside of the envelope must indicate the bidder’s name, license
number, expiration date, and that part of classification applying to the bid in accordance
with TCA 62-6-119. Bids not conforming to these provisions shall not be opened.

Bids will be received at the following address:

City of St. Joseph – P.O. Box 37 – 409 N Main Street- St. Joseph, TN 38481 until 3:00
p.m. September 4th, 2018.

Each bidder agrees by the submission of his bid to commence work within fifteen (15)
days of the issuance by the City of a “Written Notice to Proceed” and to fully complete
the work within forty five (45) calendar days from the date of the Notice to Proceed.

The successful bidder will be required to furnish a Performance and Payment Bond.
Installation shall meet TOOT Standard Specifications for Road and Bridge Construction.

No bidder may withdraw his bid within thirty (30) days after the date of the opening
thereof.

The work shall have a one-year warranty secured by a bond.

Bid opening will occur immediately at 7:00 p.m. on the submission date. Any bids
received after the scheduled submission deadline will be returned unopened to the
bidder. Fax bids will not be accepted. The City of St. Joseph does not discriminate on
the basis of age, race, sex, color, national origin, religion or disability in admission to,
access to, or operation of its programs, services or activi ties, nor does it discriminate in
its hiring or employment practices.

Copies of the Specifications and Contract Documents packet are available by contacting
the City at (931) 845-4141. The City of St. Joseph reserves the right to reject any and/
or all proposals and to award the bid in any manner deemed to be in the best interest
of the City.

ADVERTISEMENT FOR BIDS

Sealed bids for the Bid Packages listed below for the construction of a New Summertown
High School will be received by the Construction Manager, Lashlee-Rich, Inc., at the
Summertown High School, 411 W. College Street, Summertown, TN 38483 until 2:00

P.M Local Time, Tuesday, August 28, 2018 at which time and place they will be publicly
opened and read aloud.
Bids will be received on the following Bid Packages:

Bid Package #12 – Masonry

Bid Package #43 – Plumbing

No bid will be received or accepted after the above specified time for the opening of
bids. Such bids may be deemed invalid and returned unopened to the Bidder.

Copies of the Contract Documents (one (1) set maximum) may be obtained for bidding
purposes by qualified Bidders at the office of Lashlee-Rich, Inc., 1100 W. Main,
Humboldt, TN. 38348 upon deposit of a check for $300.00 made payable to Lawrence
County Schools.

The full amount of the deposit will be refunded to all bidders upon the return of the
Drawings and Project Manual to the Office of the Construction Manager in good
condition within ten (10) days after the opening of bids

Each Labor and Material or Labor Only Bid, in excess of $25,000.00, must be
accompanied by a Bidders Bond executed by the Bidder and a surety company
authorized to transact business in the State of Tennessee in the sum of not less than five
percent (5%) of the total amount of the bid as a guarantee that, if the bid is accepted,
the required sub-contract will be executed and the required performance and payment
bonds furnished.

Said bond or check will be returned to the unsuccessful bidders as soon as the contract
has been awarded, and the successful bidder as soon as he has executed the subcontract
and furnished the necessary bonds.

The successful bidder for each Labor and Material or Labor Only Bid Package in
excess of $25,000.00, will be required to furnish a satisfactory Performance Bond and
Payment Bond in the full amount of the bid. Such bond shall be from a surety company
authorized to transact business in the State of Tennessee. Attorneys-in-Fact who sign
any bonds must file with each instrument a certified and effective dated copy of their
Power of Attorney.

All bidders must be licensed contractors as required by the Contractors Licensing Act
of 1976, enacted by the General Assembly of the State of Tennessee on March 21,
1976, as currently amended, TCA 62-6-119. The Bidder’s name, license number, date
of expiration of license, and license classification must be placed on the outside of the
envelope containing the bid; otherwise, by State Statute, the bid cannot be opened or
considered.

Effective January 1, 2011, a masonry subcontractor must be licensed with an “LMC”
classification in order to bid or to be listed on the outside of bid envelope as a Licensed
Masonry Contractor (LMC) when the masonry portion is $100,000 or more (including
materials and labor). The BC-9 or BC will not be acceptable.

All Labor and Material and Labor Only Bidders shall attach the Drug Free Work Place
Affidavit (Refer to document 00 22 17- Drug Free Affidavit) to their Bid.

All Bidders shall attach the Iran Divestment Act Certification (Refer to document 00 22
19 – Iran Divestment Act Certification) to their Bid

No bidder will be permitted to withdraw his bid within 60 days after the date of opening
of bids.

The Owner reserves the right to waive any informalities or to reject any or all bids, to
evaluate bids and to accept any bid which, in its opinion, may be for the best interest
of the Owner.

No sub-contract is given or implied to the successful bidder until a written sub-contract
is offered by the Construction Manager and signed by all parties.

Successful bidders shall be required to comply with Public Chapter No. 587 / Senate Bill
No. 2048 enacted by the General Assembly of the State of Tennessee which amends
Tennessee Code Annotated, Section 49-5-413, relative to contracting with certain
persons who may have contact with certain children, enacted July 1, 2007.

All bidders shall be required to comply with provisions of Tennessee Code Annotated
§§ 12-12-101 –12-12-113 (Iran Divestment Act) enacted by the General Assembly in
the year 2016.

Bids may be faxed to the office of the Construction Manager, 1100 East Main Street,
Humboldt, TN 38343, (#731-784-6483). Faxed bids must be date stamped no later
than 10:00 A.M. on Tuesday, August 28, 2018. Faxed bids date stamped after 10:00

A.M.will not be considered. For faxed bids requiring Bid Bonds and/or Drug Free
Workplace Affidavits, these documents must be faxed with the bid.

Pursuant to section 66-31-105(2) (G), Tennessee Code Annotated, notice is hereby given that the contents of space number 10 in the self-service storage facility known as Ethridge Mini Storage will be sold at auction to the highest bidder for cash at 10:00 a.m. on Saturday, September 1, 2018 at Ethridge Mini Storage, Lawrence County, Tennessee, to satisfy an owner’s lien on such contents, claimed by Kayle Hutchens. Said self-service storage facility is located at the address set forth above where the sale will take place. The name of the occupant (lessee) of said space number: 10 is Kayla Hutchens.
